CHRONIC LOWER BACK pain?

I have somthing wrong with my back for about 4 years I can afford to see a doctor to get pain medication (ULTRAM) but due to no med insurance I have been taking these meds for that period of time taking 10-15 daily. due to tolerance built up. I live in Russellville kentucky can anyone direct me to a med facility that will treat my back because i fear i’m killing myself with pain meds HELP!!! thanks
